Job Purpose
Care Coordinator will collaborate closely with the RN and the Director of clinical operations at ANEW to ensure the effective coordination of physical and medical care services. This role is pivotal in overseeing medication administration and facilitating the scheduling of essential medical appointments for clients.
Duties/Responsibilities:
- Medication Administration:
- Administer medications to clients according to prescribed guidelines and protocols.
- Maintain accurate records of medication distribution and ensure compliance with regulatory standards.
- Monitor and report any medicationrelated concerns or issues to the Program Supervisor.
- Inventory Medications
- Review after visit summaries and new orders
- Medical Appointment Coordination:
- Assist clients in scheduling and coordinating medical appointments, including consultations, tests, and followup visits.
- Communicate effectively with healthcare providers, ensuring timely and accurate transmission of relevant client information.
- Facilitate transportation arrangements as needed for clients attending medical appointments.
- Client Support and Advocacy:
- Provide ongoing support and guidance to clients in navigating healthcare systems and accessing necessary services.
- Advocate on behalf of clients to ensure their needs are met and their rights are upheld within the healthcare setting.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that address clients' medical and psychosocial needs.
- Other Duties:
- Complete Health Intake form with new admits
- Approve medical passes
- Work collaboratively with pharmacy and ANEW medical partners
- Train and onboard new Recovery Advocates in QuickMAR and medication administration
- Maintain ongoing, current, and required training and selfdevelopment for best practice
