Location: Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte.
This role is ideal for an experienced or new graduate social worker looking to join a collaborative team of case management professionals.

What You'll Do
Participate in complex discharge planning process as a valued member of the healthcare team
Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans
Collaborate with RN Case Managers for guidance with medically complex cases
Serve as a resource for discharge planning and care coordination for psychosocially complex cases, working with MSW Case Managers as needed
Responsible for facilitating coordination of care
Maintain current knowledge of available community resources and post acute care options
Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health
What We're Looking For
Education : Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education is required
Experience : Two or more years professional social work experience or two or more years healthcare experience preferred
Case Management or related experience preferred
Additional Skills (required): Weekend and holiday coverage.
Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
